To get a consistent Goregasm combo, purchase and upgrade the autoshotgun to the maximum. All you need is one hit on most zombies to kill them, giving you one shot, one kill capability.

Activate Dual Wield in a profile where it's been unlocked, then quit and restart a new game. You will be able to play Dual Wield mode in the new profile.

The infamous lost golden brain changes location, though this is not a glitch, the developers just decided to make achieving everything close to impossible. The lost golden brain appears as just a speck in the corner of the pool room on the far right side, it is found when you turn around after viewing the "experiments" and Isaac says "You're into some sick s**t man."

Kill 50+ mutants with your Wii remote held sideways in one level to get an odd bonus of $1337.
